STEP ONE
- Our receptionist will get some basic information from you. At this point you will learn about the benefits that SALSA offers - a complete aftercare program of patient education and support. We'll call you back after we have conducted a preliminary review of your insurance.

STEP TWO
- We will request you complete a Patient Registration Packet. The packet can be mailed, faxed, or we will direct you to an address to download it online. The packet must be submitted to us (with every line filled-in) to determine if an appointment for an initial consultation can be made.

STEP THREE
- You will receive a letter confirming the time and date of your appointment, with instructions. You will be given a tremendous amount of information at this appointment, it is a vital step in you learning what the surgery does and does not do. At this appointment we will get a history of your health, later followed-up with a phone call from our Registered Nurse.

STEP FOUR
- At your initial consultation you will receive a listing of lab tests and clearances you will be required to obtain. Once EVERY item from the listing is completed and submitted, we will review it to determine if surgery is a feasible and safe option for you. If we determine you are a candidate and your insurance has been authorized, we will contact you to make an appointment for an office visit.

STEP FIVE
- The surgeon will review the lab results and clearances with you:
Attend Office Visit
- If the surgeon clears you for surgery
- If payment source has been verified.
- If the necessary releases and agreements are signed and on file
You may be scheduled for surgery within three months.
- If the surgeon reviews your situation and determines that more evaluation is indicated.
- You will be given instructions.
Possibly a follow-up office visit will be scheduled.
